A recreational runner who recently ran in a major marathon visits your sports medicine clinic. She is complaining of what appears to be a skin lesion, consisting of a small, thickened area of skin, on the bottom of her right foot, just under the ball of her foot (distal head of the 1st metatarsal). On closer inspection you find the following: she claims the area is painful when walking or running and the lesion appears to have a tiny dark red dot within it.
