The National Center for Chronic Disease Prevention and Health Promotion (NCCDPHP) was established in 1989 to consolidate Centers for Disease Control and Prevention (CDC) efforts in chronic disease prevention and health promotion. The vision of the Center is to enable all people to lead long, healthy, satisfying lives.
Visit the NCCDPHP web site. Select and click on "Overview." After reading about chronic disease, select "CDC's Chronic Disease Programs." From the list presented choose one major chronic disease and, in your own words, write a one-page report of some of the major risks attributed to the group and/or condition chosen.
